What to Bring to Your Appointment
To ensure your consultation runs smoothly and efficiently, please bring the following items:
- Referral letter from your GP
(Required for Medicare claims)
- Relevant imaging and reports
X-rays, scans, and any other documentation from your GP.
- If your imaging was done at Queensland X-Ray NQ X-Ray or Imed Radiology , Dr Hazratwala can access it directly.
- If performed elsewhere, please inform our staff when making appointment , so prior arrangements can me made to access the images.
- If images cannot be viewed, new scans may be required, which could incur additional costs.
- Medication history
- Private health insurance details
- WorkCover claim details (if applicable)
- Support person
Family or friends are welcome to assist with discussion and decision-making.
- A written list of questions
This helps ensure all your concerns are addressed.

To cancel an appointment:
Please telephone the office during business hours. Please cancel at least one day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
