What To Bring To An Appointment
When you come for your appointments, please remember to bring the following:
- Your referral letter from your GP (without this referral you will not be able to claim through Medicare)
- X-rays, scans and any other information that you have from your GP
- Medication history
- Health insurance details
- Workcover claim details, if applicable
- Any family members or friends you may want to have present to help in the discussion and decision making process
- A written list of questions you may have
If your scans/x-rays were done at Qld X-Ray imaging group then Dr Hazratwala can access your images directly from his computer as the Qld Xray system is linked. However if you had scans/x-ray done elsewhere you may be required to bring hard copies as the disk may not work on Dr Hazratwala’s computer system. If you have concerns regarding your imaging, please ask one of our staff members for further clarification. If Dr Hazratwala is unable to view your images he may have to order new scans/images which may incur an unnecessary cost to you.
To cancel an appointment:
Please telephone the office during business hours. Please cancel at least one day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
